Job Description
- Evaluate patients and perform appropriate therapy interventions
- Develop a comprehensive plan of care and coordinate multi-disciplinary care
- Perform prompt clinical documentation and visit notes
- Continued communication with physician, care team members, patient and family
- Comfortable working in all aspects of the home health setting
- Manage patient treatment and outcomes directly, or through COTA supervision
Requirements
- Strong organization skills and time management
- Able to multitask and accomplish and meet timelines
- Professional appearance and effective communication skills
- Current Texas Occupational Therapist (OT) license
- One year of occupational therapy experience
- Current Texas driver's license
- CPR certification
- Experience with healthcare software (Homecare Homebase preferred)
- Previous Home Health experience is helpful, but not required
