Seeking Occupational Therapists in the Home Health setting to cover Crosby territory and its surrounding areas.

Per-Visit Rates:

  • $75 per regular OT Visit
  • $112.50 per OT Eval
  • $187.50 per OT SOC

Job Description:

  • Evaluate patients and perform appropriate therapy interventions
  • Develop a comprehensive plan of care and coordinate multi-disciplinary care
  • Perform prompt clinical documentation and visit notes
  • Continued communication with physician, care team members, patient and family
  • Comfortable working in all aspects of the home health setting
  • Manage patient treatment and outcomes directly, or through COTA supervision

Requirements:

  • Strong organization skills and time management
  • Able to multitask and accomplish and meet timelines
  • Professional appearance and effective communication skills
  • Current Texas Occupational Therapist (OT) license
  • One year of occupational therapy experience
  • Current Texas driver's license
  • CPR certification
  • Experience with healthcare software (Homecare Homebase preferred)
  • Previous Home Health experience is helpful, but not required

Schedule: Monday to Friday

License/Certification:

  • BLS Certification (Required)
  • Occupational Therapy License (Required)
