Per Visit Rates: $75 per regular ST Visit; $112.50 per ST Eval

Job Description:

  • The Speech-Language Pathologist is responsible for comprehensive assessments in the patient’s home, including the completion of skilled documentation through Homecare Homebase.
  • Evaluate patients and perform appropriate therapy interventions.
  • Develop a comprehensive plan of care and coordinate multi-disciplinary care.
  • Perform clinical documentation and visit notes.
  • Continued communication with physician, care team members, patient and family.
  • Comfortable working in all aspects of the home health setting.
  • Implement and maintain SLP protocols.
  • Manage patient treatment and outcomes directly.

Requirements:

  • Current Texas Speech-Language Pathology License.
  • One year of SLP experience.
  • Experience with Homecare Homebase is helpful, but not required.
  • Previous Home Health experience is helpful, but not required.
  • Current Texas driver’s license.
  • CPR certification.
