The Forward Trust is seeking a Continuity of Care Coordinator to join our substance misuse team at HMP Brixton. Based primarily at Brixton, you will help strengthen the journey from prison into community treatment and build effective partnerships to ensure a safe, supported transition for people leaving custody.
You will work across London services, including HMP Wormwood Scrubs and Heathrow IRC, with travel as required.
