You will support adults 18+ with drug/alcohol challenges as part of an integrated healthcare team, delivering person-centred support and helping clients progress along their recovery journeys.
Key duties include caseload management, assessments, care planning, harm reduction, and coordinating access to housing, benefits, employment and healthcare services.
