Transforming Support provides person centred supported living and outreach services for adults with learning disabilities, autism and complex needs across the UK. The Role As a Support Worker, you will provide consistent, person centred support that promotes dignity, independence, choice and involvement in the local community. Your responsibilities will include:
- Supporting individuals with everyday living and maintaining their independence
- Helping people make choices about their daily routines, activities and lifestyle
- Supporting access to the local community, appointments, education, employment and social activities
- Providing support with health needs and medication in line with agreed procedures
- Helping individuals manage their tenancy, finances and household responsibilities where required
- Building positive relationships with the people you support, their families and other professionals
