Hours: 40 hours per week, Monday to Friday (between 8:00am and 6:00pm)

Salary: £31,262.40 per annum

Work Location: Huddersfield Head Office

We are looking for someone with:

  • Strong written communication skills and the ability to produce clear, structured documentation

  • The ability to handle confidential information appropriately
  • Proficiency in Microsoft Office (Word, Excel, Outlook, PowerPoint)
  • A full UK driving licence and access to a car insured for business use

Role overview:

Working with the Support Planning Manager, this role involves undertaking care needs assessments and working collaboratively with clients, families, professionals, and colleagues to develop person-centred support plans. The role also includes organising and attending 6-weekly and annual service reviews to ensure clients achieve their maintenance and personal outcomes.

Additional responsibilities include:

  • Developing and maintaining a Menu of Wellbeing Enriching Activities
  • Supporting managers and teams to create tailored activity plans
  • Establishing an Activity Coordinator Champions Network
  • Coordinating group activities and events for all clients, including those attending the Day Centre
