Active Prospects is seeking an experienced and compassionate Care Manager to lead our complex care service in Redhill, Surrey. This is an exciting opportunity for someone with strong experience supporting people with learning disabilities, autism, and complex needs, including behaviours that may challenge.
The service currently supports three amazing people with autism, communication and sensory needs, and some behaviours that challenge. They require support with all aspects of daily life, including personal care, medication, meals, household routines, and taking part in recreational activities that bring them joy and fulfilment.
Our aim is to empower the people we support to live ordinary lives through active support, positive risk-taking, creative thinking, and a person-centred approach. At present, all the people we support receive 2:1 support during the day and 1:1 support at night.
We are looking for someone who is passionate about empowering people to live fulfilling and aspiring lives. You will be kind, patient, resilient and compassionate, with a strong belief in the value of excellent leadership. This role can be challenging at times, but the difference you will make for the people we support and the team you lead is incredibly rewarding.
You will be adaptable, flexible and confident in leading staff teams to support people to build independence, make choices, and achieve positive outcomes through empowerment, active support, positive risk-taking and creative, person-centred practice.
