Service Type: Supported living, one property for one Gentleman.
Speciality: Learning Disability, Mental Health and Autism (behaviours of concern, complex care, self‑harm and forensic support).
This role supports Adam to leave a long‑stay hospital and rebuild his life in the community. It requires robust experience in learning disability and autism care, with a passion for empowering people and working within an integrated multidisciplinary approach. You should be kind, patient, resilient, compassionate, proactive, and a strong leader. The role can be tough, but the rewards outweigh the challenges.
Key responsibilities include:
- Leading a small team to provide daily support, positive behaviour support, risk management and life‑skill development for Adam.
- Coordinating with external services, healthcare professionals and family to ensure a seamless support network.
- Planning and monitoring service delivery, maintaining accurate records, and progressing staff development.
- Managing budget, resources, and ensuring compliance with policy and law.
