Active Case Management is seeking three or four dynamic female support workers to provide support in everyday activities to a friendly, outgoing, mature lady with physical disabilities and cognitive impairments, as a result of an acquired brain injury and dementia., Under the supervision of the Clinical Case Manager, to be part of a team providing support with everyday activities, personal care, medication administration and assistance with rehabilitation for a client in her own home. Working as a member of a 2:1 support team, meeting the needs of the client as requested and in line with her Care Plan and Support & Rehabilitation Programmes., · Maintaining the client’s safety and well-being at all times in accordance with care plan, risk assessments and direction, but also using initiative where necessary.

· Communicate and interact with client, listen to her wishes and act on these as appropriate.

· Medication management.

· Completing daily reports, monitoring forms and handover documentation.

· Attending regular 1:1 supervision with the Team Leader and/or Clinical Case Manager.

· Attending staff meetings and training.

· Manual handling.

PERSONAL CARE DUTIES:

· Assisting with personal care; showering, bathing, brushing teeth.

· Dressing.

· Make-up application and hair styling.

· Support with feeding.

· Assistance with toileting.

DOMESTIC SUPPORT DUTIES:

· Maintaining the client’s nutritional needs – shopping and preparing food.

· Undertaking light household duties i.e. tidying and cleaning to ensure the client is cared for within safe, hygienic surroundings, and that the working environment complies with Health and Safety Regulations

· Laundry tasks.

REHABILITATION SUPPORT DUTIES:

· Working in line with the guidance, instructions and training received from the rehabilitation team.

· Providing support to the client and therapists during therapy sessions.

SOCIAL AND LEISURE SUPPORT DUTIES:

· Travelling with, acting as driver and accompanying the client on community activities and holidays.

· Being actively involved in activities, at the client’s request.

· Helping the client to participate in a variety of leisure and recreational activities, motivating and encouraging where appropriate.

· Promoting visits and communication from the client’s family members and friends.

· Assist with generating new ideas for activities and trips out, pro-actively seeking opportunities and helping with the planning process, including holidays., Enhanced Disclosure and Barring Service (DBS) check. This ensures compliance with the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975. This includes a check of the Adult Barred List. Candidates must declare any unspent criminal convictions during the application process. This is in line with our commitment to safeguarding the welfare of vulnerable adults under our care.

We are looking for someone that meets the following essential criteria:

· Full driving licence.

· Previous work experience as a carer/support worker with adults.

· Basic level of IT skills, including completing electronic records, sending emails and attaching documents.

· Compassionate and person-centred attitude.

It would be desirable if you also have:

· Previous experience of working with adults with an acquired brain injury and/or dementia.

· Experience of working with clients in their own homes., * GCSE or equivalent (preferred)

WORKING HOURS: We have the following permanent hours available and work on a two-week rolling rota, however specific days and shift times will be agreed with the successful candidate:

  • Day Shift: 9.15am – 4.00pm Tuesday, Wednesday, Friday, Sunday

  • Day Shift 3.45pm – 10.00pm Monday through to Friday and Sunday

  • Sleep-In Shift: 9.40pm – 9.30am Monday, Tuesday, Sunday, Pension and Benefits Information: This role includes auto-enrolment into a workplace pension scheme, in line with statutory requirements. We also offer additional benefits such access to the Employment Assistance Programme.

Job Types: Full-time, Part-time

Pay: £14.00-£15.00 per hour

Expected hours: No more than 40 per week
