Brownbill Case Management has been established since 1992 and exists to deliver an outstanding case management service to children, young people and adults who have enduring and complex health conditions and injuries. As part of the Active Care Group (ACG) Case Management Division, Brownbill Case Management Services (BAL) provides a specialist case management service primarily for individuals who have been affected by brain injury, spinal cord injury, Cerebral Palsy, complex orthopaedic injuries and amputation. We work across the Midlands, London and Home Counties, and South East regions.

The primary role of the Clinical Case Manager is to network cost effective services and resources on behalf of our clients to bring about an improvement in their quality of life. This includes assessing, facilitating, planning and advocating for their needs on an individual basis. This ensures that management is Client focused; that the most appropriate services are accessed; and that continuity, consistency and structure are maintained. Our work includes setting rehabilitation goals with Clients and support workers in the home environment; facilitating the client's and family's adjustment to the injuries; recruiting, training and supporting support workers; and managing care regimes.

Ideally:

  • Full time, 35 hours per week / negotiable for an Associate Case Manager
  • Mon to Fri office hours
  • Hybrid
  • Our coverage is London & Home Counties, Midlands, Kent, Essex and the South coast, candidates from these locations would be highly desirable

Some of the main roles and responsibilities include:

  • Provide & maintain a case management service to a range of clients, families & Finance Deputies working within all Brownbill Policies, Procedures & supporting documentations.
  • Assessment of Client Needs and Risks and Produce Case Management Plan and agree with the client and/or their representatives prior to providing active intervention.
  • Produce action plans and goals with timeframes that are specific and relevant to the client.
  • Establish and maintain a positive therapeutic working relationship with clients, families, Deputies and litigation solicitors.
  • Produce and implement a Support & Rehabilitation Plan with specific guidelines for support workers to implement the support/rehabilitation program.
  • Recruit, select & manage care/support workers.
  • To undertake any other duties appropriate and as reasonably required, to meet the changing needs of the service.
  • Ensuring service user needs and business objectives are met.

What you'll have:

  • Hold a UK professional Health qualification ie, nurse, OT, Physio Social Worker etc.
  • BABICM / IRCM / CMSUK registration desirable
  • Have some experience of neurological conditions and working in a community setting.
  • Thorough knowledge of the CQC fundamental standards and quality statements.
  • Full working knowledge of the Mental Capacity Act (MCA), Deprivation of Liberty Safeguards (DoLS) and Safeguarding frameworks.
  • Ability to work as part of a team and as an individual.
  • To provide direction support workers.
  • Self-confidence and able to manage challenging situations.
  • Ability to address and respond to employee issues in a fair and consistent manner, including informal and formal performance management.
  • Excellent interpersonal, verbal and written communication skills.
  • Strong organisational, time management and planning skills, with a commitment to self-management and continuous personal development.
  • IT literate. Use of MS Word, Excel and Outlook.
  • Flexible, professional and positive approach to multi-site working.
  • Proven reliability in terms of full attendance and timekeeping.
  • Full UK driving licence.
