Westcountry Case Management is a specialist, independent case management company supporting children, young people and adults who have sustained catastrophic injuries, including acquired brain injury, spinal cord injury and other complex neurological conditions. Our experienced team works closely with clients, families, legal professionals and multidisciplinary rehabilitation teams to deliver high-quality, person-centred case management services that maximise independence, wellbeing and quality of life.
Due to continued growth, we are looking to expand our network of Associate Clinical Case Managers across the Southampton region. This is an excellent opportunity for experienced clinicians and established case managers seeking flexible work alongside a respected and supportive specialist case management provider.
About the Role
As an Associate Clinical Case Manager, you will manage your own caseload while benefiting from the support, expertise and professional reputation of Westcountry Case Management. You will work collaboratively with clients, families, deputies, solicitors and treating professionals to assess needs, coordinate rehabilitation programmes and ensure clients have access to the services and support required to achieve their rehabilitation goals. Our associates play a vital role in helping clients rebuild their lives following life-changing injury, delivering tailored rehabilitation and support solutions within their home and community environments.
What you'll be doing:
- Undertake comprehensive assessments of client needs, strengths, risks and rehabilitation requirements.
- Develop and implement person-centred Case Management and Rehabilitation Plans.
- Coordinate and monitor multidisciplinary rehabilitation programmes.
- Establish positive and effective working relationships with clients, families, deputies, solicitors and treating professionals.
- Facilitate goal setting, outcome monitoring and regular review processes.
- Source, recruit and oversee rehabilitation assistants and support teams where appropriate.
- Produce high-quality reports, records and professional documentation.
- Ensure all interventions comply with relevant legislation, professional standards and best practice guidance.
- Contribute to positive outcomes that promote independence, participation and quality of life.

What you'll have:
- Professional registration in a relevant health or social care profession (Nursing, Occupational Therapy, Physiotherapy, Speech and Language Therapy, Social Work or equivalent).
- Experience working with individuals with acquired brain injury, spinal cord injury, complex trauma or neurological conditions.
- Strong understanding of rehabilitation pathways and community-based service delivery.
- Excellent communication, relationship-building and report-writing skills.
- The ability to work independently and manage a diverse caseload.
- Strong organisational and time-management skills.
- A full UK driving licence and willingness to travel as required.
Desirable:
- BABICM membership or accreditation.
- Previous case management experience.
- Experience working within medico-legal or litigation-funded rehabilitation services.
- Established professional networks within health, social care and rehabilitation services.
