Westcountry Case Management is a specialist, independent case management company supporting children, young people and adults who have sustained catastrophic injuries, including acquired brain injury, spinal cord injury and other complex neurological conditions. For over 25 years, we have worked alongside clients, families, legal professionals and multidisciplinary teams to coordinate rehabilitation, maximise independence and improve quality of life. As our services continue to grow, we are seeking an experienced and motivated Clinical Case Manager to join our team across the Plymouth region.

About the Role

As a Clinical Case Manager, you will work closely with clients and their families to assess needs, coordinate rehabilitation programmes and develop tailored support packages that promote independence, wellbeing and meaningful outcomes. You will take a lead role in identifying and sourcing appropriate services, coordinating multidisciplinary input and ensuring that rehabilitation goals remain focused on each client's individual needs and aspirations. What you’ll be doing

  • Undertake comprehensive assessments of client needs, strengths, risks and rehabilitation requirements.
  • Develop and implement person-centred Case Management and Rehabilitation Plans.
  • Coordinate and monitor multidisciplinary rehabilitation programmes.
  • Establish positive and effective working relationships with clients, families, deputies, solicitors and treating professionals.
  • Facilitate goal setting, outcome monitoring and regular review processes.
  • Recruit, induct, supervise and support rehabilitation assistants and support workers where appropriate.
  • Ensure all interventions are delivered in accordance with current legislation, professional standards and best practice guidance.
  • Maintain accurate records, reports and documentation to a high professional standard.
  • Contribute to service development and continuous quality improvement initiatives.

What you’ll have

We are keen to hear from experienced clinicians who are passionate about supporting people with complex injuries and disabilities within their own homes and communities. You will have:

  • Professional registration in a relevant health or social care profession (Nursing, Occupational Therapy, Physiotherapy, Speech and Language Therapy, Social Work or equivalent).
  • Experience working with individuals with acquired brain injury, spinal cord injury, complex trauma or neurological conditions.
  • Strong knowledge of rehabilitation principles and community-based practice.
  • Understanding of safeguarding, the Mental Capacity Act and relevant statutory frameworks.
  • Excellent communication, organisation and report‑writing skills.
  • The ability to work independently while contributing positively to a multidisciplinary team.
  • A full UK driving licence and willingness to travel across the South West region.

Desirable

  • BABICM membership or accreditation.
  • Previous case management experience.
  • Experience working within medico‑legal or litigation‑funded rehabilitation services.
