We are looking for an outstanding Spinal and Neuro National Referrals Nurse with advanced experience in spinal neuro‑care and neuro‑rehabilitation to join our team in a pivotal, service‑leading role. This is a national travelling position, critical to ensuring high quality clinical assessment, seamless admissions, and excellent ongoing relationships with clients, families, and admitting services.
You will be the clinical face and voice of our service, working with clients who have complex spinal neurological conditions and ensuring their needs are understood and met across multiple settings. You will work closely with multidisciplinary clinical teams and external services to support continuity of care. When not travelling to assess patients you will be a key part of client relationship management and quality.
What you’ll be working: Our standard working hours for this role are 37.5 hours per week, Monday to Friday. Remote working with frequent travel nationally to ACG sites.
What you’ll be doing: The National Referrals Nurse is critical to safeguarding quality, continuity, and experience for people living with complex neuro needs. We know that high‑quality neuro rehabilitation goes beyond standard care — it involves extensive assessment, engagement, and tailored planning that supports recovery and meaningful life outcomes. Providers like Active Neuro are transforming neuro‑rehabilitation through outcomes‑focused care and multidisciplinary support combining clinical assessment with real‑world rehabilitation pathways. This role ensures that clients are referred and admitted into appropriate services safely, efficiently and with full clinical context, setting them up for optimal care right from the first contact.
- Expert Clinical Assessment: Conduct detailed, person‑centred, in‑person assessments for adults with complex neurological conditions including acquired brain injury, spinal injury, stroke, degenerative conditions, epilepsy and more — mirroring a neuro‑rehabilitation approach. Evaluate clinical needs, functional capacity, risks and rehabilitation potential, using professional judgement and evidence‑based assessment frameworks.
- Coordination & Communication: Build strong, trusting relationships with clients, families and carers to understand their goals, priorities and support needs. Prepare and deliver coherent, comprehensive clinical referrals and summaries to admitting services so that all key information is communicated clearly and actionably. Advocate for clients in discussions with healthcare providers, funders and external professionals.
- Admission Facilitation & Handover: Support clients through the admission process, ensuring admitting teams understand medical history, clinical needs and social context. Attend MDT meetings, admission planning calls, and transition briefings where needed.
- Quality & Clinical Governance: Maintain accurate clinical records and risk documentation to the highest standard. Work within regulatory and professional frameworks, ensuring compliance with best practice and quality standards.
- Relationship Management: Ensure clients relationships with the service is going smoothly and they are getting everything they need from the service. Ensure clients are getting all set therapy and they have no concerns regarding their time in the service – dealing with concerns immediately if they arise. Working closely with the concierge service at all times, providing leadership around admissions; including likes/dislikes of clients.
What you’ll have:
- Current NMC registration (Registered Nurse)
- A legal right to work in the U.K.
- Experience in private care or hospital environment
- Experience in community neuro‑rehabilitation
- Proven spinal/ neuro clinical assessment experience
- Experience liaising with NHS/ICB commissioners
- Exceptional communication and documentation skills
- Experience with outcome measurement and goal setting
- Ability to work independently
- Experience in MDT leadership or coordination
- National travel flexibility
- Specialist training or certification in neurological care
- Understand business development and relationship management in detail and be aware of the relationship between revenue and admissions.
- Have strong clinical expertise in neurological assessment, rehabilitation principles and risk management.
- Demonstrate exceptional communication and relationship‑building skills across a wide range of stakeholders.
- Is confident working autonomously and making advanced clinical decisions.
- Have experience with challenging or medically complex cases and coordinating care across multidisciplinary teams.
- Is highly organised, detailed, and professional in documentation and reporting.
- Is willing and able to travel nationally to conduct assessments and attend meetings.
