We have wonderful opportunities for a full time Rehabilitation Assistant to join our caring and supportive nursing team delivering individualised care to patients (including personal care), implementing person-centred care plans and undertaking a broad range of caring duties.

Blackheath is a 36 bed centre providing specialist person-centred rehabilitation, care and support for adults with either a brain injury or other complex neurological condition. We provide comprehensive assessment, rehabilitation and therapy and specialise in the treatment of patients with a primary cognitive or behavioural impairment resulting from a brain injury or other neurological condition.

What you'll be working: 37.5 hours per week. Alternate working pattern 3 days one week and 4 days next week.

What you'll be doing:

  • Assist nursing staff in providing high quality individualised care to a clearly defined group of patients, relatives and carers
  • Work and act as a key member of the team, supporting each patient in achievement of their planned outcomes
  • Build relationships with patients, which are open and two way
  • Support patients to undertake their rehabilitation programme
  • Establish methods of communication with patients, reducing barriers when necessary
  • Support with activities of daily living and personal care when needed
  • Communicate any issues to the clinical team
  • Accurately report and/or record work activities
