Northern Case Management has been established since 1997 to support people with acquired neurological damage. We are seeking an Associate Clinical Case Manager to join our team, focusing on Paediatric and/or Adult cases in the Bury and North West area.
You will assess needs, plan and advocate for clients, set rehabilitation goals with families and support workers, recruit and supervise staff, and manage care regimes to maintain continuity and quality of life.
