We are seeking a highly organised and motivated individual to join our Group Learning Support team as a Transition Coordinator on a maternity cover basis. In this role, you will play a key part in ensuring the smooth transition of all learners with an Educational, Health and Care Plan (EHCP) into and out of college, delivering high-quality support interventions that enhance their retention and overall achievement.

This is a fixed-term, full-time position working 37 hours per week, based at our Bracknell College, with the flexibility to work across our Berkshire campuses. The contract will run from June 2026 to August 2027. Hybrid working may be permitted during term time, subject to agreement with line management.

What do you need to be successful as an EHCP Transition Coordinator at Activate Learning?

  • You will have experience of working with learners who have an Educational Health and Care Plan (EHCP) and have a wide knowledge of disabilities and learning difficulties.
  • Excellent interpersonal and communication skills, demonstrating effective professional relationships with colleagues, learners and the wider community are essential.
  • Knowledge and understanding of the SEND Code of Practice and other relevant legislation is essential.
