The Action Together team is vital in achieving the charity's purpose - to maximise opportunities to create positive social change; promote social justice and harness social benefit; and to strengthen the voluntary, community, faith and social enterprise sector. We're looking for people who share our values to join our team.
This role will join our established social Prescribing team to empower people living in Oldham to take an active role in improving their own health and wellbeing. By working alongside individuals, Primary Care teams, and community organisations, the postholder will co‑produce personalised wellbeing action plans, strengthen community connections, and ensure people can access the right non‑clinical support at the right time.
As a key member of the Oldham Social Prescribing Service, the postholder will manage a caseload of clients, contribute to MDT discussions, identify those who would benefit from social prescribing, and support the development of targeted approaches for priority groups. The ideal candidate will be someone who recognises that many of the barriers to wellbeing can be addressed through strong community connections, and who has the passion and skills to help people access the support around them. You will be a confident collaborator who can build effective relationships across agencies, work independently, and manage a busy caseload. With experience in strengths‑based work, community‑based or remote support, and partnership working, you will bring a solid understanding of the health and social care landscape and the role of social prescribing within it.
