Age UK Salford and Trafford is seeking a passionate Coordinator on a maternity cover basis to deliver a strengths-based approach to our Age Well Early Intervention Neighbourhood Model, supporting Aftercare Services for Salford residents aged 55+ who are either struggling at home or have recently been discharged from hospital.
The postholder will work with colleagues and external agencies to improve patient flow, reduce the risk of hospital readmission, and promote independence and wellbeing.
