The Growth Company’s Employment team is seeking a Work and Health Coach to engage directly with participants and provide tailored support that helps individuals stay in work, return to employment following sickness absence, or move into new roles. This position plays a key part in empowering people to overcome barriers, build confidence, and progress toward sustainable employment outcomes.
The coach will deliver the Employability service in MFT in the community, applying the principles of personalised information, advice and guidance (with full training provided). They will also connect participants with local and public services that promote improved health, wellbeing, mental health, and employment readiness, ensuring each person receives holistic, joined‑up support.
Key Responsibilities- Manage a caseload of voluntary participants, supporting their wellbeing, employment goals, and progression toward work by addressing biopsychosocial barriers.
- Conduct Work and Health Assessments in person or via Microsoft Teams, identifying support needs such as benefits guidance, mental health disclosures, and workplace considerations.
- Create personalised Action Plans that help participants understand their skills, aspirations, and goals, enabling them to secure, sustain, or return to employment.
- Source suitable job opportunities through tailored job searches and proactive engagement with local employers, exploring both advertised and hidden vacancies.
- Work collaboratively within a multi‑disciplinary team and build strong partnerships with clinical staff, GP practices, Talking Therapies, MFT services, social prescribers, the VCF sector, employers, and Jobcentre Plus.
- Deliver triage, make referrals to external services where needed, and provide employer‑focused support such as negotiating adjustments, return‑to‑work strategies, and job‑retention guidance.
- Maintain accurate data entry, case records, monitoring information, and monthly case studies while adhering to safeguarding, information governance, and administrative protocols, whilst working to individual targets.
- Participate in continuous learning and mandatory training, attend internal and MFT meetings, and stay informed on best practice in vocational rehabilitation.
- Understanding of how physical and mental health conditions can affect a person’s ability to work, along with awareness of local health services.
- Knowledge of the challenges faced by individuals with multiple or complex barriers to gaining and sustaining employment.
- Familiarity with support agencies that assist with issues such as debt, substance misuse, housing, and other complex needs.
- Awareness of a broad range of occupations and the local labour market to guide participants effectively.
- Ability to build rapport through a warm, empathetic, and approachable style that supports participant engagement and progression.
- Strong relationship‑building skills to work collaboratively with customers and partner organisations.
- Demonstrates excellent interpersonal and communication abilities, building rapport with a wide range of individuals.
