We have a rewarding Support Worker vacancy at our Brunswick Day Centre in central Manchester. You\'ll be part of a care team supporting our clients to make the most of later life.

  • PURPOSE OF JOB
  • To care for older people in the Day Centre as part of a care plan intended to help the users of the service achieve maximum independence, observing the practices and procedures set out in the Age Concern Manchester Handbook.
  • To assist in the provision of a flexible transport service for older people to and from the Day Centres, driving the minibus as required.
  • To work with other members of the care team to help maintain and improve the quality of care provided in the community for individual users.
  • To ensure that the Equal Opportunities and Health and Safety policies of Age Concern Manchester as they apply to the Day Centre situation are adhered to.
  • To provide relief cover for other staff as required.
  • TASKS
  • To provide care and support to older people with complex mental and physical health problems attending the Day Centre in accordance with the practices and procedure set out in the Age Concern Manchester Handbook.
  • To support the Day Centre Management team with the provision of all activities relating to the Day Centre service.
  • To maintain a high standard of care, which responds to individual, assessed needs and that is recorded within the care plan.
  • To assist with the provision of assessment, care planning and monitoring of the Care Plan; liasing with other professionals within the Day Centre network.
  • To develop warm and trusting relationships with clients to encourage them to express their needs, views and concerns reporting any changes in their physical and mental state to the Day Centre Management team.
  • To monitor the mental and physical health of clients receiving the Day Care service and accompany clients to GP appointments and hospital reviews where appropriate.
  • Maintain client information in accordance with Day centre recording systems and policies, ensuring confidentiality is maintained at all times.
  • To participate in staff/ volunteer training and development including team meetings as appropriate.
  • To follow Health and Safety guidelines carefully and alert the Day Centre Management team immediately of any concerns in relation to Health and Safety issues.
  • To work with and provide support to volunteers within the Day Centre.
  • To assist the Day Centre Management team with the provision of a flexible transportation system which meets the needs of clients receiving Day Care services
  • To work flexibly and provide relief cover within the organisation as required.
  • Closing Date: Friday 28th August 2026 at 12:00pm
  • Closing Time: 5pm
  • Working Pattern Details: 35 Per Week normally Monday to Friday, 8.30 am–4.30 pm excluding lunch.
