Community Navigation Service

The Tameside Live Well Community Navigation Service helps adults aged 18 and over access the support, opportunities and connections they need to live well. Delivered in communities across Tameside and funded by Tameside Council, the service supports people who want to make positive changes to their health, wellbeing, or circumstances by connecting them with the right help at the right time.

Our independent Community Navigators work alongside local residents, providing as much or as little support as needed. With detailed knowledge of services, activities and community resources across Tameside, the team helps individuals find personalised support that matches their needs, goals and interests. Whether someone is looking to improve their wellbeing, access practical help, build confidence, reduce isolation, or become more connected to their community, we help them navigate the options available. By building strong links with local statutory services, community organisations and voluntary groups, Community Navigation makes it easier for residents to access support and prevent issues from escalating.

Alongside direct support for individuals, the service also invests in community development to strengthen local networks, identify gaps in provision and ensure communities have the resources needed to meet the changing needs of Tameside residents.

Focused on what matters most to the people we work with, the service helps individuals discover opportunities, build meaningful connections and access the support they need to live well.

The Role

Live Well Community Navigation Service Administrator

The Community Navigation Service Administrator is the ‘glue’ that holds the service together working between our clients, staff, internal teams and referrers across Tameside. You will be the first point of contact into the service for clients, triaging cases and helping people with queries; and the ambassador for this and our wider Action Together Offer.

The ideal candidate

The ideal candidate will be an empathetic, inclusive and highly motivated individual with excellent communication and interpersonal skills, committed to achieving the best outcomes for the people they support. They will be well organised, able to manage multiple priorities effectively, and demonstrate a strong attention to detail. They will be confident using case management systems, maintaining accurate records and communicating professionally with a wide range of stakeholders, including members of the public, colleagues, health and care professionals and referral agencies.
