We connect people with what’s happening in their community, develop community ideas into action, strengthen local organisations, and provide strategic influence for the charity and voluntary sector. The Action Together team is vital in achieving the charity's purpose – maximising opportunities to create positive social change, promoting social justice and harnessing social benefit, and strengthening the voluntary, community, faith and social enterprise sector.

Join the established social prescribing team to empower people living in Oldham to take an active role in improving their own health and wellbeing. Co‑produce personalised wellbeing action plans, strengthen community connections and ensure people can access the right non‑clinical support at the right time. Manage a caseload of clients, contribute to multidisciplinary team discussions, identify those who would benefit from social prescribing and support the development of targeted approaches for priority groups.

Ideal Candidate

  • Recognises that many barriers to wellbeing can be addressed through strong community connections.
  • Confident collaborator who can build effective relationships across agencies, work independently and manage a busy caseload.
  • Has experience in strengths‑based work, community‑based or remote support, partnership working and a solid understanding of the health and social care landscape and the role of social prescribing within it.
