Age UK Salford and Trafford are seeking a passionate and committed Coordinator to join us on a maternity cover basis. The role will lead a coordinated, strengths-based approach to delivering our Age Well Early Intervention Neighbourhood Model. This role is responsible for supporting the development and delivery of high-quality Aftercare Services for Salford residents aged 55+ who are either struggling at home or have recently been discharged from hospital.

The postholder will work to improve patient flow, reduce the risk of hospital readmission, and promote independence and wellbeing. Key responsibilities include:

  • Providing practical and person-centred support
  • Facilitating access to relevant health and social care services
  • Working in partnership with colleagues and external agencies across the continuum of care to ensure a joined-up and responsive service

The role also involves embedding effective working practices and systems, ensuring delivery aligns with agreed outcomes and key performance indicators, and contributing to the ongoing development and improvement of the service. The role focuses on building strong relationships with internal teams and external partners to support collaborative working and deliver positive outcomes for older people across Salford.

This is a fixed-term position for up to 12 months to provide maternity cover, with the contract ending on the return of the substantive postholder.
