Monmouthshire Domiciliary Care, commissioned by Monmouthshire Council, offers consistent, person‑centred support to one young person with a disability, helping them build independence, manage daily routines, and feel confident in their home environment.
A bit about the role:
As a Housing Support Practitioner you will provide professional oversight to a small team of Housing Support Workers. This part‑time leadership role is ideal for someone with strong practice knowledge, excellent communication skills, and a passion for ensuring high‑quality, person‑centred support. You will play a key role in maintaining the quality, consistency, and safety of the support provided. You will guide and mentor the Housing Support Workers, oversee rota management, and ensure that the young person’s needs remain at the centre of everything we do.
What you do could make a difference:
- Providing professional oversight and guidance to Housing Support Workers
- Leading on rota management, ensuring consistent staffing and appropriate cover
- Reviewing support plans, risk assessments, and daily notes for accuracy and quality
- Offering reflective supervision, coaching, and practice support to staff
- Ensuring safeguarding procedures are followed and concerns are escalated appropriately
- Liaising with families, social workers, therapists, and other professionals
- Monitoring the young person’s progress and adapting support strategies as needed
- Ensuring compliance with organisational policies and regulatory standards

- Level 3 qualification in Health & Social Care (or equivalent)
- An understanding of safeguarding, risk management, and person‑centred practice
- Experience supporting children, young people, or adults with disabilities
- Experience in domiciliary care, supported living, or a similar setting

- Excellent communication, organisational, and record‑keeping skills
