Are you passionate about empowering older people to live independently, stay connected, and access the support they deserve? We are seeking a compassionate and proactive Intake Assessment Coordinator to be the welcoming first point of contact for clients and families entering our Support at Home programs.
What you’ll be doing:
- Leading intake & assessments – processing My Aged Care referrals, conducting face‑to‑face assessments, and ensuring every client receives a respectful, person‑centred entry experience.
- Building strong partnerships with hospitals, pharmacies, service providers, and community organisations to strengthen access and collaboration.
- Supporting client independence by identifying needs, setting goals, and ensuring timely commencement of services.
- Maintaining high‑quality documentation that meets Aged Care Quality Standards and Support at Home compliance.
- Contributing to continuous improvement through data insights, reassessments, and collaboration with Managers and Clinical teams.
