Location: Walsall
The Intermediate Care Team at Walsall Council/NHS is seeking an experienced Senior Physiotherapist. This role will support individuals to maximise their independence through rehabilitation, rapid response, and reablement services across community and bed-based pathways.
Main Duties:
- Provide physiotherapy assessment and clinical expertise across all Intermediate Care pathways including reablement, rapid response, and rehabilitation beds.
- Support individuals to regain independence through rehabilitation, mobility assessments, and goal‑focused interventions.
- Work collaboratively within a multidisciplinary team (MDT) including health and social care professionals.
- Manage and prioritise a varied caseload across community and intermediate care settings.
- Deliver person‑centred rehabilitation programmes to support recovery and independence.
- Contribute to discharge planning and prevent unnecessary hospital admissions where possible.
- Provide advice, guidance, and support to colleagues within the multidisciplinary team.
- Support integrated working across Walsall Council and NHS services.
Requirements:
- Diploma/Degree in Physiotherapy.
- HCPC Registration.
- Car driver with access to a vehicle.
- Post‑graduate physiotherapy experience (candidates should have some experience beyond newly qualified level due to the complexity of the caseload).
- Experience working within a multidisciplinary team and delivering rehabilitation.
- Community experience is essential.
- Strong clinical assessment and rehabilitation skills.
- Excellent communication, organisational, and clinical decision‑making abilities.
