Location: Walsall
Walsall Council / NHS – Intermediate Care Team is seeking an experienced Senior Occupational Therapist to join their Intermediate Care Team. This role will support individuals to maximise their independence through rehabilitation, rapid response, and reablement services across community and bed-based pathways.
Main Duties- Provide occupational therapy assessment and clinical expertise across all Intermediate Care pathways including reablement, rapid response, and rehabilitation beds.
- Support individuals to regain independence through functional assessments, rehabilitation, and goal‑focused interventions.
- Work collaboratively within a multidisciplinary team (MDT) including health and social care professionals.
- Assess individuals’ ability to complete activities of daily living and recommend appropriate interventions, equipment, or adaptations.
- Manage and prioritise a varied caseload across community and intermediate care settings.
- Deliver person‑centred rehabilitation programmes to support recovery and independence.
- Contribute to discharge planning and help prevent unnecessary hospital admissions where possible.
- Provide advice, guidance, and support to colleagues within the multidisciplinary team.
- Support integrated working across Walsall Council and NHS services.
- Diploma/Degree in Occupational Therapy.
- HCPC Registration.
- Car driver with access to a vehicle.
- Post‑graduate occupational therapy experience (candidates should have some experience beyond newly qualified level due to the complexity of the caseload).
- Experience working within a multidisciplinary team and delivering rehabilitation.
- Community experience is essential.
- Strong knowledge of functional assessment, rehabilitation, and promoting independence.
- Excellent communication, organisational, and clinical decision‑making abilities.
