We are currently recruiting an experienced Social Worker to join the Adults Locality Team within Wiltshire Council. This is an exciting opportunity to join a well‑structured, supportive and fast‑paced team that is expanding its workforce. Wiltshire is embedding a systemic approach and is undergoing positive changes focused on reducing caseloads and improving outcomes for individuals and families. You will benefit from a strong supervision culture, flexible working arrangements, and a collaborative team environment.
Key Responsibilities
- Work closely with professionals across health, voluntary and statutory sectors as part of a multi‑disciplinary team, ensuring effective communication and shared best practice.
- Review, monitor and evaluate care plans and the quality of service provision, responding proactively to changing needs.
- Develop, implement and assess reablement programmes within care packages, promoting independence and positive outcomes.
- Manage your own caseload or specialist projects, ensuring appropriate interventions and timely outcomes.
- Complete risk assessments, safe methods of work practice guides, and raise awareness of a wide range of care‑related issues and their implications.
- Undertake visits across the community, including home visits, as required.
Requirements
- Qualified Social Worker with relevant experience in Adult Social Care.
- Full UK driving licence and access to a vehicle (or ability to travel via other suitable means).
- Ability to work flexibly, with the role being predominantly office and community‑based, alongside some home working.
