We are seeking an experienced Senior Social Worker to join the Wolfson Neuro‑Rehabilitation Unit at Queen Mary’s Hospital. This is a key role for a professional with a strong background in adult social care, hospital discharge planning, and neuro‑disability. The Wandsworth Hospital Team works closely with the Wolfson Unit to provide a high‑quality, person‑centred discharge service that promotes independence and well‑being for patients and their carers. You will support individuals with complex physical and cognitive needs and play a central role in longer‑term care planning.
What We’re Looking For:
- Qualified Social Worker registered with Social Work England.
- Extensive post‑qualification experience in adult social care, ideally within hospital or neuro‑rehabilitation settings.
- Proven ability to manage complex cases and lead discharge planning.
- Experience supervising staff and contributing to service improvement.
- Strong knowledge of relevant legislation and statutory responsibilities.
- Excellent communication, decision‑making, and organisational skills.
- Collaborative approach with confidence in working across health and social care disciplines.
Key Responsibilities:
- Provide supervision and support to Social Workers in the team.
- Lead complex discharge planning and support patient flow within the Wolfson Unit.
- Undertake assessments, manage risk, and make evidence‑based decisions.
- Act as safeguarding enquiry officer and manage new referrals.
- Attend and contribute to multidisciplinary meetings.
- Support service development and contribute to quality assurance and performance monitoring.
