Main Duties:

  • Review existing care and support packages for adults living in Supported Living properties
  • Apply a strength‑based and incremental approach to assessments
  • Complete Care Act 2014 assessments
  • Assess service users in the community and recommend appropriate interventions
  • Provision of equipment and minor adaptations
  • Undertake moving and handling assessments where required
  • Work collaboratively with social workers, providers, families, and other professionals
  • Ensure service users’ needs are met while promoting independence, dignity, and safety
  • Work in line with safeguarding policies and local authority procedures

Requirements:

  • Minimum 2 years post‑qualified experience
  • Previous experience working as a community‑based Occupational Therapist
  • Strong understanding of the Care Act 2014
  • Experience evaluating care and support packages
  • Manual handling expertise
  • HCPC Registration
  • Enhanced DBS/CRB (assistance available)
  • Eligibility to work in the UK
  • Ability to work effectively as part of a local authority team
