Reablement Worker

Hammersmith and Fulham are looking to recruit an enthusiastic Reablement Worker to join a well-structured and dynamic team. The client offers a full-time contract, a fair and manageable caseload and further training in your portfolio.  The vacancy will be for 12 months with potential for extension.Main Duties for the Reablement Worker includes.

• Provision of care and support, and advise.to enable residents to meet the outcomes of their personalised care and support programme.
• To observe, monitor and report to the resident’s GP, District Nursing team, reablement colleagues and any other relevant professionals. any significant changes in the residents’ health, mood, behaviour, attitude, or changes in circumstances.
• Under the supervision of the reablement professionals’ or involved health professionals to implement individual programmes, record changes and monitor progress.
• To be available via work mobile phone during working hours and respond promptly to communications with the Reablement service and residents as necessary.
• To work within Care Quality Commission (CQC) and Safeguarding requirements and have a willingness to undertake training within the agreed timeframes, to ensure that you are up to date with any changes to legislation or required.
• To attend and participate in Multi-disciplinary Team (MDT) meetings
• To support residents, their friends, carer, or family member to make a complaint or feedback about the service.
• To undertake moving and handling and assist residents with transfers with the use of specialist equipment (hoist etc.) and specific adaptations.
• To carry out duties as directed and assessing staff in reablement with task which are consistent with the duties and responsibilities of the post.

To Be Successful.

• Previous experience working with adults with physical disabilities
• Enhanced DBS/CRB (we can obtain for you)
• Eligibility to work in the UK
• Occupational Therapist Qualification
