We are looking to recruit a Qualified Social Worker in the Hospital Discharge Team within Islington. The successful candidate will be working in a well-structured, organised team who are growing their social worker staffing levels. The responsibilities of the Social Worker include:

  • Assess the social care needs of adults in an acute hospital setting to support timely, safe discharge
  • Complete Care Act assessments, mental capacity assessments, and best interest decisions in line with legislation
  • Work collaboratively with health professionals, families, and partner agencies to coordinate discharge planning
  • Arrange appropriate packages of care, placements, and community support services
  • Safeguard adults at risk and respond to concerns in line with statutory duties and local procedures
  • Contribute to multidisciplinary team meetings and discharge planning forums
  • Maintain accurate, timely, and legally compliant case records
  • Promote strengths-based, person-centred practice and support service user choice and independence
  • Manage a complex caseload, prioritising effectively to meet service demands and discharge targets.

The ideal candidate will have:

  • Social Work England registration
  • Eligibility to work in the UK
  • Social Work qualified (MA/BSc/DipSW/CQSW/MSc/BA)
