Job Description

We are seeking experienced and proactive Occupational Therapists to join our community-based Adult Social Care team in East Cornwall, with the flexibility to work across the county alongside two other therapists. This is a vital role focused on delivering face-to-face, person-centred interventions that promote independence and reduce reliance on care packages supporting Cornwall Council’s 2025/26 savings targets.Key Responsibilities:

  • Carry out holistic assessments of adults with complex physical, sensory, and cognitive needs.
  • Develop and implement goal-oriented care plans aimed at promoting independence in the community.
  • Recommend and facilitate the provision of equipment, minor and major adaptations, and assistive technology.
  • Maintain regular face-to-face contact with service users.
  • Collaborate with multi-disciplinary teams, service users, families, and carers.
  • Accurately maintain case records in accordance with legal and professional standards.
  • Actively contribute to safeguarding and risk management processes.
  • Promote and work in accordance with The Care Act, Mental Capacity Act, and other relevant legislation.

Requirements:

  • HCPC registered Occupational Therapist
  • Substantial experience in community settings, ideally within Adult Social Care
  • Strong understanding of relevant legislation and the ability to apply clinical reasoning in complex cases
  • Comfortable managing a caseload independently
  • Willing and able to conduct weekly face-to-face visits
  • Enhanced DBS (or willing to obtain)
  • UK right to work & relevant Occupational Therapy qualification
