Ackerman Pierce is recruiting for 1 Community Care Officer position within the Occupational Therapist Assistant - Hybrid – Community based visits plus working from either office base or home depending on task.. This opportunity is suited to a committed professional who can support occupational therapy and community-care activity through practical assessment work, coordinated interventions and accurate follow-up.
The successful candidate will join an established local-authority service and work with colleagues, individuals, families, carers and partner organisations as appropriate to the assignment. The emphasis will be on timely, well-reasoned practice, clear communication and achieving safe, person-centred outcomes.
Key responsibilities
- Support qualified practitioners with allocated assessments, visits and agreed follow-up activity.
- Gather relevant information about daily living, mobility, the home environment and identified support needs.
- Assist with equipment, adaptation or independence-focused recommendations within delegated responsibilities.
- Communicate clearly with individuals, carers, colleagues, providers and partner services.
- Recognise and escalate safeguarding or risk concerns promptly.
- Maintain accurate records and progress actions within agreed timescales.
Candidate profile
- Relevant experience in social care, community support, rehabilitation or an occupational-therapy-related environment.
- A practical, person-centred approach and an understanding of promoting independence and safety.
- Good observation, communication and record-keeping skills.
- Able to work reliably within delegated authority while seeking guidance when appropriate.
Service information and working arrangements
Occupational Therapist Assistant
Hybrid – Community based visits plus working from either office base or home depending on task.
- To work under direction of qualified Occupational Therapist and carry out therapy plans with people of Hertfordshire. This could include any activities or daily living.
- Minimum of 2 new cases a week, interventions may last up to 12 weeks
