Community Team in Slough looking to recruit a Discharge to Assess Occupational Therapist to join a supportive and fast-paced team. This is a fantastic opportunity to be part of a highly collaborative service focused on enabling timely and safe discharges from hospital. The role offers a full-time locum contract at £36.80 per hour, a fair and manageable caseload, and strong multi-disciplinary support.

Main Duties for the Discharge to Assess Occupational Therapist include:

  • Supporting the Discharge to Assess pathway by facilitating efficient hospital discharges.
  • Working closely with the Hospital Social Work Team to assess patient needs in the community.
  • Delivering timely occupational therapy assessments and interventions to promote independence at home.
  • Managing a caseload within a community setting as part of a team of three experienced OTs.
  • Ensuring service users and their families are fully engaged in the discharge planning process.
  • Applying sound clinical reasoning and maintaining accurate, up-to-date documentation.

To Be Successful:

  • Previous experience working as an Occupational Therapist in a hospital discharge or community rehab setting.
  • Ability to work autonomously and hit the ground running.
  • HCPC Registration.
  • Enhanced DBS/CRB (we can obtain this for you).
  • Eligibility to work in the UK.
  • Recognised Occupational Therapy qualification.
