Location: Bexley
Main duties for the D2A Occupational Therapist include:
- Undertake strengths-based occupational therapy assessments under the Discharge to Assess pathway
- Support safe and timely discharge from hospital
- Assess functional ability, risk, and environmental factors in community settings
- Recommend and arrange equipment, adaptations, reablement, or short-term support
- Work collaboratively with health partners, reablement teams, and social care colleagues
- Contribute to decision-making around longer-term care and support planning
- Maintain accurate records in line with statutory and professional requirements
To Be Successful:
- Previous experience working as an Occupational Therapist in the community
- HCPC Registration
- Enhanced DBS/CRB (we can obtain for you)
- Eligibility to work in the UK
- Occupational Therapist Qualification
