Ackerman Pierce is recruiting for 2 Adult Social Worker positions within Hertfordshire County Council. This opportunity is suited to a committed professional who can deliver person-centred assessment, care planning and safeguarding activity that promotes independence, wellbeing and positive outcomes for adults.
The successful candidate will join an established local-authority service and work with colleagues, individuals, families, carers and partner organisations as appropriate to the assignment. The emphasis will be on timely, well-reasoned practice, clear communication and achieving safe, person-centred outcomes.
Key responsibilities
- Undertake Care Act assessments, reviews and proportionate reassessments according to service priorities.
- Develop outcome-focused care and support plans with adults, families, carers and partner professionals.
- Complete safeguarding work, risk assessments and Mental Capacity Act activity where required.
- Coordinate effectively with health, housing, voluntary-sector and other multidisciplinary services.
- Maintain accurate records and progress casework in line with statutory duties, policy and agreed timescales.
- Promote choice, control, independence and the least restrictive approach throughout practice.
Candidate profile
- A qualified Social Worker with the professional registration required for the assignment.
- Sound knowledge of adult social care legislation, safeguarding and person-centred practice.
- Able to manage complex situations, make defensible decisions and communicate clearly with all parties.
- A reliable and organised practitioner who can manage competing priorities and work constructively within a multidisciplinary service.
Service information and working arrangements
On site SW post at based at Wellington House Assessments in persons home or in a place. The post will have a focus on Care Act assessments, safeguarding, MCA’s, Reviews and Domestic DoLS
Social Work Team – Specialist Community Disability Service 25+
Ideally would need to be a car driver as may need to travel, however not essential.
