Job Description
- To represent my team in triaging and collaborating with other professionals like the OT and doctors to triage cases appropriately.
- ?To represent my team in MDT meetings to update the whole triaging team about the current state of cases that has been assessed and accepted as requiring social care intervention.
- ?To complete Care Act assessment and review of needs to identify the needs of our service-user and provide the right interventions. Example, packages of care.
- ?To make referrals to other Agency for support. Like Age UK, Assistive technology support,
- ?To complete mental capacity assessment under the MCA 2005
Experience preferred / Minimum Requirements:
- Post qualifying experience of working as a Hospital or Initial Response Social Worker
- SWE Registration
- 1 year post qualifying experience within UK Social Care industry
- Enhanced DBS/CRB (we can obtain for you)
- Eligibility to work in the UK
