Triage & Response Social Worker We are looking to recruit a Qualified Social Worker – Triage & Response team with the Social Care industry in Bristol City Council. The Qualified Social Worker – Triage & Response team vacancy in Bristol City Council will be for an initial 3-6 months with potential for extension subject to internal circumstance and your performance. We are seeking dedicated and compassionate Social Workers to join the discharge teams at Southmead and Bristol Royal Infirmary Hospitals and employed by Bristol City Council. You will have a passion for enabling service users to leave the hospital in a timely manner with appropriate support in place. Social Workers will work in coordination with health colleagues in discharge teams to provide appropriate, strengths-based, and timely support. Responsibilities include completing Care Act assessments, mental capacity assessments, and other assessments in line with legislation, developing comprehensive discharge plans, and collaborating with medical staff to ensure seamless patient care transitions.
