An experienced Occupational Therapist is required to support a review team as part of a time-limited project in Rochdale. Main Duties:
- Review existing care and support packages for adults living in Supported Living properties
- Apply a strength-based and incremental approach to assessments
- Complete Care Act 2014 assessments
- Assess service users in the community and recommend appropriate interventions
- Provision of equipment and minor adaptations
- Undertake moving and handling assessments where required
- Work collaboratively with social workers, providers, families, and other professionals
- Ensure service users’ needs are met while promoting independence, dignity, and safety
- Work in line with safeguarding policies and local authority procedures
Requirements:
- Minimum 2 years post-qualified experience
- Previous experience working as a community-based Occupational Therapist
- Strong understanding of the Care Act 2014
- Experience evaluating care and support packages
- Manual handling expertise
- HCPC Registration
- Enhanced DBS/CRB (assistance available)
- Eligibility to work in the UK
- Ability to work effectively as part of a local authority team
