Occupational Therapist – Reablement Team – Wiltshire Council

We are currently looking for a motivated and experienced Occupational Therapist to join the Reablement Team at WiltshireCouncil. This is a full-time, contract role focused on empowering individuals to regain their independence and remain safely at home following illness, injury, or a hospital stay. You\’ll work within a multi-disciplinary team to deliver short-term, goal-focused interventions in a fast-paced community setting.

Responsibilities

  • Undertaking strengths-based, holistic assessments of adults with a range of physical and cognitive needs.
  • Developing person-centred reablement plans aimed at increasing independence and reducing long-term care needs.
  • Providing clinical input into the use of equipment, minor adaptations, and assistive technology.
  • Supporting service users to achieve functional goals in collaboration with reablement support workers.
  • Working closely with social workers, GPs, therapists, and community health professionals.
  • Reviewing progress and adjusting interventions to meet changing needs.
  • Ensuring all interventions align with the Care Act, Mental Capacity Act, and local reablement policies.

To Be Successful

  • Experience working as an Occupational Therapist in reablement, intermediate care, or community settings.
  • Knowledge of functional assessment tools and short-term interventions.
  • Ability to set SMART goals and promote independence.
  • HCPC Registration
  • Enhanced DBS/CRB (we can obtain this for you)
  • Eligibility to work in the UK
  • Recognised Occupational Therapy qualification
