Occupational Therapist – Reablement Team – Wiltshire Council
We are currently looking for a motivated and experienced Occupational Therapist to join the Reablement Team at WiltshireCouncil. This is a full-time, contract role focused on empowering individuals to regain their independence and remain safely at home following illness, injury, or a hospital stay. You\’ll work within a multi-disciplinary team to deliver short-term, goal-focused interventions in a fast-paced community setting.
Responsibilities
- Undertaking strengths-based, holistic assessments of adults with a range of physical and cognitive needs.
- Developing person-centred reablement plans aimed at increasing independence and reducing long-term care needs.
- Providing clinical input into the use of equipment, minor adaptations, and assistive technology.
- Supporting service users to achieve functional goals in collaboration with reablement support workers.
- Working closely with social workers, GPs, therapists, and community health professionals.
- Reviewing progress and adjusting interventions to meet changing needs.
- Ensuring all interventions align with the Care Act, Mental Capacity Act, and local reablement policies.
To Be Successful
- Experience working as an Occupational Therapist in reablement, intermediate care, or community settings.
- Knowledge of functional assessment tools and short-term interventions.
- Ability to set SMART goals and promote independence.
- HCPC Registration
- Enhanced DBS/CRB (we can obtain this for you)
- Eligibility to work in the UK
- Recognised Occupational Therapy qualification
