Slough are looking to recruit an enthusiasticD2A Occupational Therapist to join a well-structured and dynamic team. The client offers a full time contract with an opportunity to work from home, a fair and manageable caseload and further training in your portfolio. The vacancy will be for an initial 3 ā 6 months with potential for extension.
Responsibilities
Undertake strengthsābased occupational therapy assessments under the Discharge to Assess (D2A) pathway
Support safe and timely discharge from hospital
Assess functional ability, risk, and environmental factors in community settings
Recommend and arrange equipment, adaptations, reablement, or shortāterm support
Work collaboratively with health partners, reablement teams, and social care colleagues
Contribute to decisionāmaking around longerāterm care and support planning
Maintain accurate records in line with statutory and professional requirements
Qualifications
Previous experience working as an Occupational Therapist in the community
HCPC Registration
Enhanced DBS/CRB (we can obtain for you)
Eligibility to work in the UK
