The London Borough of Havering is looking to expand its current team with a dedicated and experienced Advanced Practitioner seeking an opportunity to join a well-structured and organised Leaving Care Service. As well as joining a supportive and ambitious team, our client also offers a competitive salary, opportunities for ongoing training to enhance your skills, and clear progression pathways within the service. This is a fantastic opportunity to work with an established team and partner agencies to provide high-quality support to children and young people transitioning from care to independence. This Advanced Practitioner role will initially be for 3–6 months, with potential for extension subject to internal circumstances and performance.

Responsibilities include:

  • Supporting and mentoring Social Workers and Personal Advisors within the Leaving Care Service.
  • Holding a reduced caseload of complex young people and providing practice leadership across the team.
  • Completing high-quality Needs Assessments, Pathway Plans, risk assessments, and statutory visits for young people leaving care.
  • Ensuring all key performance indicators are met, including pathway plans, reviews, case recordings, chronologies, and health-related assessments for young people aged 16+.
  • Working in partnership with housing, education, health, and external agencies to achieve positive outcomes for care leavers.
  • Promoting independence, safeguarding, and effective transition planning for children and young people leaving care.
  • Contributing to service development and maintaining high standards of practice across the team.

Experience preferred:

  • Post-qualifying experience of working within a statutory Leaving Care or Looked After Children Team within the UK Social Care sector.
  • Experience supporting less experienced staff, supervising practice, or deputising for management where required.

Minimum Requirements:

  • Social Work England Registration
  • Minimum 5 years’ post-qualifying experience within UK Children’s Services
  • Enhanced DBS/CRB (we can obtain for you)
  • Eligibility to work in the UK
