Achieving for Children Community Interest Company is seeking a Preparing for Adulthood (PfA) Coordinator to support young people with SEND as they prepare for adulthood. You will ensure EHCPs remain effective, coordinate phase transfers, and work with families, schools, social care, health and other professionals to deliver statutory duties and positive outcomes.
You will manage Annual Reviews, advise on participation and mental capacity, and promote safeguarding and inclusive practice.
