As a Preparing for Adulthood (PfA) Coordinator, you'll support young people with SEND as they prepare for adulthood, ensuring their Education, Health and Care Plans (EHCPs) remain effective and reflect their changing needs. You'll coordinate phase transfers and transitions, working closely with young people, families, schools, colleges, Social Care, Health and other professionals. You'll manage and lead Annual Reviews, oversee changes to EHCPs and ensure appropriate transition plans are in place in line with statutory requirements. You'll also provide advice around participation and mental capacity, while promoting good practice and helping young people achieve positive outcomes as they move into adulthood.
About you
- Degree-level qualification or equivalent relevant experience in education, teaching, children's services, social work, disability, psychology or a related field.
- Strong knowledge of SEND legislation and the SEND Code of Practice, particularly Preparing for Adulthood.
- Good understanding of EHCPs and the Education, Health and Care services involved in their delivery.
- Experience of multi-agency working, including with Adult Social Care, education, health and vocational services.
- Knowledge of pathways and support available for young people who are NEET or whose EHCP is coming to an end.
- Excellent written communication skills, with the ability to produce clear, accurate and high-quality statutory documents.
- Ability to manage competing priorities, meet statutory deadlines and work effectively in a fast-paced environment.
- Confidence working independently, using initiative and sound judgement to resolve complex cases.
- Strong understanding of safeguarding, equality, disability and inclusive practice.
- Ability to make responsible and accountable decisions involving public funding.
