Job Description
We are looking for a highly skilled and motivated Assessment & Placement Manager to join our community hub team. The successful candidate will play a crucial role in supporting the organisation\’s core operations and ensuring the efficient and effective delivery of services.
Key Responsibilities:
- Complete high-quality initial assessments for individuals, presenting suitable support options aligned with their needs and aspirations.
- Articulate clear outcomes with and for the individual, focusing on providing the best quality support.
- Deliver target occupancy and fee growth across an allocated area.
- Collaborate with fellow APMs to achieve individual and team occupancy targets.
- Develop key relationships with families, health, and key stakeholders to promote Achieve together, establishing the organisation as a trusted provider and encouraging referrals.
- Network with local Care Managers and Commissioners to identify opportunities for new referrals and developments.
- Act as an ambassador for Achieve together, promoting the organisation at events such as Transition Fairs.
- Maintain a focus on the core vision of supporting people to have happy, healthy, and meaningful lives.
Requirements:
- Strong understanding of sector trends and strategic development plans of purchasers.
- Experience with brokerage, purchasing systems, and commissioning portals.
- Knowledge of the Learning Disability sector, including legislative and CQC regulatory requirements.
- Understanding of the structure of the sector, including residential and supported living settings.
- Knowledge of how local authorities, councils, and ICBs commission and agree on services.
