Leaving Care Support and Integration Officer (18+)

Location: Bristol

This is a full-time, 1-year fixed-term contract, working 37.5 hours per week.

Our mission goes beyond individual support; we actively challenge and disrupt the systems that perpetuate inequalities in our society. With a proven track record of delivering effective support services, in 2024 we helped house over 1,400 individuals and provided support to over 500 service users, helping them achieve personal goals and build fulfilling lives in their new country.

What You’ll Be Doing

  • Provide tailored support to young people aged 18+ who have left the care system and are living in supported housing.
  • Manage a caseload of care-experienced young people, complete person-centred support plans and risk assessments, and offer guidance on housing, health, finance, education and employment.
  • Support individuals through one-to-one work and regular reviews to develop life skills, build social networks, and access opportunities and services to achieve personal goals.
  • Help young people access legal services where required.
  • Signpost to external agencies and develop partnership links with organisations.
  • Keep accurate case records and manage information using our CRM system.
  • Attend meetings, training and supervision sessions.

What We’re Looking For

  • Experience working with young people in a support service setting.
  • Experience providing advice, guidance, advocacy and culturally sensitive support.
  • Experience creating support plans and completing needs and risk assessments.
  • Ability to maintain accurate records and manage a caseload.
  • Strong organisational and planning skills.
  • Confident IT skills, including Outlook, Excel, Word and PowerPoint.
  • Strong verbal and written communication skills.
