About the Role
- Undertake initial holistic nursing assessments in partnership with the service user and their family.
- Develop individualised care plans in partnership with the service user, their families and other health care professionals to ensure goals are patient focused, realistic and achievable.
- Produce standard and bespoke risk management plans for service users, ensuring any identified high risk is escalated and acted upon, working with other stakeholders to plan, monitor and review effectiveness of any plan in place.
- Be responsible for the delivery of client specific clinical training for staff within packages.
- Be accountable for ensuring all clients allocated are regularly reviewed in accordance with company policies, ensuring high standards of clinical practice and reflecting current organisational and national guidelines and standards.

- A Learning Disability or Mental Health Nursing degree with an active NMC pin.
