Are you an experienced Advanced Nurse Practitioner with a passion for mental health? Do you want to be part of a progressive, supportive and patient‑centred team making a real difference in the community? St Helens South PCN is seeking a dynamic ANP with expertise in mental health to join our growing multidisciplinary team.
About the Role
You will be at the heart of delivering high‑quality proactive mental health care across our PCN. Working closely with GPs, social prescribers, care coordinators and community mental health teams you will play a vital role in early intervention, holistic assessment and ongoing management of patients with mental health needs.
Key Responsibilities
- Conduct comprehensive mental health assessments and formulate care plans
- Prescribe and review mental health medications within scope of practice
- Support patients with complex mental health presentations
- Lead on mental health pathways and contribute to service development
- Liaise with local mental health services and voluntary sector partners
- Provide training and support to primary care colleagues on mental health matters
About You
- Registered Nurse with valid NMC registration – Essential
- MSc or equivalent qualification in Advanced Clinical Practice – Essential
- Independent prescriber (V300) – Essential
- Significant experience in mental health settings (primary, secondary or community)
- Confident working autonomously and as part of a multidisciplinary team
- Passionate about improving mental health outcomes in primary care
